Join Us for an ISPAD 2024 Session on a Data-Driven Diabetes Care Model for Pediatric Patients

If you are attending ISPAD 2024, we are excited to invite you to a workshop on:

“Enhancing Outcomes, Experience, and Efficiency through a Technology- and Data-Driven Diabetes Care Model in Pediatrics.”

Technology advancements and evolving guidelines are opening new opportunities for managing pediatric patients with Type 1 diabetes.  These innovations have the potential to significantly enhance patient experience, satisfaction, and outcomes while also providing healthcare professionals (HCPs) with valuable patient data that is often not fully utilized. By effectively harnessing these large amounts of data, we can improve the quality of care, increase clinical efficiencies, and address staffing challenges, ultimately enhancing care delivery for pediatric patients.

CloudCare is an example of a digital remote population management solution developed by clinicians for clinicians, which integrates daily sensor data from various technologies with comprehensive patient context information. This integration allows for managing the entire patient population by intelligently processing and incorporating data into clinical workflows. As a result, care teams can efficiently screen and risk-stratify patients, ensuring they receive the right care at the right time.

The population management-driven approach supports data-driven, personalized care tailored to each patient’s unique needs. It promotes a hybrid diabetes care model by effectively combining remote consultations with in-person visits.

Join our workshop to discuss how a remote population management-driven diabetes care model can enhance patient outcomes, experience, and overall efficiency.

In this workshop, our experts among with the audience, will discuss the potential benefits, risks, and practical implications of applying our technology- and data-driven diabetes care model in a pediatric setting:

  • Dr Fiona Campbell – Clinical Director, General Paediatrics/Medical Specialties and Pediatrician at the Leeds Teaching Hospital Trust NHS, UK
  • Dr Roosje Roelants – Pediatrician at Diabeter, Netherlands
  • Christine Fransman – Research Program Coordinator at Diabeter, Netherlands
  • Laya Rojas – Cloud Nurse at Sant Joan de Déu Hospital in Barcelona, Spain

Workshop structure:

  1. Introduction to CloudCare – A remote population management solution that enables data-driven and personalized diabetes care.
  2. How to evaluate the Impact of Remote Population Management-Driven Diabetes Care Model for Adults & Pediatric Study Design:
  • Presentation of clinical study setup and interim results (n=62, 6M FU and latest analysis results) of CloudCare in adults.
  • Overview of the study design for pediatrics and subsequent discussion.
  1. Workshop & Discussion: Facilitated by a moderator, this session will feature a dynamic discussion with key speakers.

Examples of topics:

  • Differences in the remote population management-driven care model between pediatric patients and adults, as well as across various countries.
  • What different results in the study might we observe when evaluating impact in a pediatric setting. Would we benefit in the future from different endpoints?
  • Potential challenges in implementing a remote population management-driven diabetes care model.
  • What is the largest win from such model from a pediatric patient perspective?
  • Best practices for ensuring smooth implementation and integration of this new care model.
  • Successful transition to a new care model through effective engagement of healthcare professionals, caregivers, and pediatric patients.
