“Enhancing Outcomes, Experience, and Efficiency through a Technology- and Data-Driven Diabetes Care Model in Pediatrics.”
Technology advancements and evolving guidelines are opening new opportunities for managing pediatric patients with Type 1 diabetes. These innovations have the potential to significantly enhance patient experience, satisfaction, and outcomes while also providing healthcare professionals (HCPs) with valuable patient data that is often not fully utilized. By effectively harnessing these large amounts of data, we can improve the quality of care, increase clinical efficiencies, and address staffing challenges, ultimately enhancing care delivery for pediatric patients.
CloudCare is an example of a digital remote population management solution developed by clinicians for clinicians, which integrates daily sensor data from various technologies with comprehensive patient context information. This integration allows for managing the entire patient population by intelligently processing and incorporating data into clinical workflows. As a result, care teams can efficiently screen and risk-stratify patients, ensuring they receive the right care at the right time.
The population management-driven approach supports data-driven, personalized care tailored to each patient’s unique needs. It promotes a hybrid diabetes care model by effectively combining remote consultations with in-person visits.

- How to evaluate the Impact of Remote Population Management-Driven Diabetes Care Model for Adults & Pediatric Study Design:
- Presentation of clinical study setup and interim results (n=62, 6M FU and latest analysis results) of CloudCare in adults.
- Overview of the study design for pediatrics and subsequent discussion.
- Workshop & Discussion: Facilitated by a moderator, this session will feature a dynamic discussion with key speakers.
